This issue may reveal a deeper confusion in the BNF.
There are two categories for identifiers associated with modules and
module instances --
module_identifier
module_instance_identifier (includes optional range expression)
In A.4.1.1 there is even a "name_of_instance".
But there is only one category for identifiers associated with interfaces
and interface instances --
interface_identifer
In A.2.1.2/A.2.3 "interface_identifier" is used for interface
instances and "identifier" is used for interfaces, but in A.1.3
"interface_identifier" is used for interfaces.
