[sv-bc] Subroutine prototype BNF fixes

Attached are BNF fixes mostly related to task/function prototypes,
especially regarding --

        function return types (see 1364-ETF issue 554)
        modport method declarations (see examples of 19.6)

It also fixes minor bugs in Syntax 23-6, incorporates a recent
fix for 1364-ETF issue 546, and ameliorates a problem related
to V2K LRM 13.
