[sv-bc] Mantis 907: Default parameter assignment should be optional

In response to valuable feedback from our recent SV-BC teleconference, I
updated the proposal for the issue mentioned in --

The updated proposal disallows omitting parameter default values except
within the "ANSI-style" #(...) declarations, and it says that a module
with a no-default-value parameter cannot be a top-level module.
