Deprecate defparam

I don't know the formal wording but I propose that we vote to deprecate
defparam statements. Verilog-2001 introduced named parameter passing which
is a much cleaner interface for passing parameters to instantiated modules.
We already have a number of restrictions on the ugly defparams with respect
to generate statements and we also documented in the Verilog-2001 standard
that two defparams made from difference files to the same parameter is
undefined. The practice of hierarchically changing parameters of another
file should be put to rest.
